某医院ICU物体表面耐药菌的分布Distribution of drug-resistance bacteria on object surfaces of ICU of a hospital

摘要(Abstract):
目的 分析ICU高频接触物体表面耐药菌分布特征,以期为指导医院消毒措施和医院感染防控提供科学依据。方法 对2021年7月-12月医院呼吸内科的ICU高频接触物体表面进行采样,使用5种抗生素培养基进行耐药菌株筛选和分离培养,采用全长16S rRNA基因的PCR扩增和一代测序技术鉴定菌种。结果 对ICU的8个高频接触点位共采集6批次48份环境拭子,共有35份样本培养阳性,总阳性率为72.92%。经筛选分离鉴定得到115株细菌,位居前三位的分别为鲍氏不动杆菌(14株,12.17%)、其次是铜绿假单胞菌(13株,11.30%)和肺炎克雷伯菌(11株,9.57%);耐药菌负荷较多的采样点是水槽存水弯(30株)、水龙头(25株)、床头柜(20株)、床栏杆(18株);水槽存水弯以铜绿假单胞菌(4株,13.33%)、肺炎克雷伯菌(4株,13.33%)的检出为主;水龙头以贝氏不动杆菌(4株,16%)、铜绿假单胞菌(4株,16%)为主;床头柜以铜绿假单胞菌(2株,10%)为主;床栏杆分离出18株耐药菌,以鲍氏不动杆菌(6株,33.33%)为主。结论 医院ICU不同物体表面的耐药菌分布存在差异,靠近水源及患者的采样位置耐药菌较多,提示医院需加强ICU物体表面消毒措施,以有效控制潜在的院内耐药菌感染及传播。
